From Referral to Your First Day

A simple, four-step process — most people hear back from us within a couple of working days.

Submit the Form

Fill out the referral form below — it takes about five minutes. Self-referrals and third-party referrals both welcome.

We Get in Touch

A member of our team calls or emails you, using whichever contact method you prefer, usually within 2 working days.

We Arrange a Visit

We meet in person to talk through goals, funding, and what support looks like day to day — no pressure, just a conversation.

You Start With Us

Once funding is confirmed, we agree a start date and build a plan around the person, not a template.

Who Can Refer?

Individuals with a learning disability, aged 18 or over, referring themselves

Family members or carers, referring on someone's behalf

Social workers and other care professionals

Funding: most people are funded through Adult Social Care via a personalised budget or direct payment card. We’re happy to talk you through what that means for your situation.
