If you have a learning disability and are over 18, you can refer yourself — or be referred by family, carers, or a social worker. We’ll take it from there.
“We’ll never ask you to explain yourself twice — tell us once, and we’ll build the plan around it.”
A simple, four-step process — most people hear back from us within a couple of working days.
Fill out the referral form below — it takes about five minutes. Self-referrals and third-party referrals both welcome.
A member of our team calls or emails you, using whichever contact method you prefer, usually within 2 working days.
We meet in person to talk through goals, funding, and what support looks like day to day — no pressure, just a conversation.
Once funding is confirmed, we agree a start date and build a plan around the person, not a template.
